Overview of NA191UPD

Web Form Review/Approval allows CEF staff to review, edit, approve, and deny web forms created by users of online (web) applications like Online-CEF. Examples of web forms include:

  • Investment purchase applications
  • Request for change of name or address
  • Investment withdrawal request


To review and process web forms:


  • From the DDI Connect main menu, double-click Name/Address System
  • Double-click Web Forms
  • Double-click Web Form Review/Approval. You will see a screen similar to this one:

If you wish to filter forms by a specific product or status, click the drop down arrow to the right of either of those fields to select your choices. Click Refresh to update the page based on your selections.


To review a form, click once to highlight and then click View, or double-click it to launch. You will now see this screen:

This screen displays three tabs: Investor, Funding and Interest.


Investor tab

On the Investor tab, the following fields are available:


Field Description
Form name Displays the name of the form being submitted
Status Displays the status (Reviewed, approved, processed, etc.)
Created Displays the creator user id, and the date and time the form was created
Reviewed Displays the reviewer user id, and the date and time the form was reviewed
Processed Displays the processing user id, and the date and time the form was processed
Account Displays the investor's account number
Account Name/Address Displays the investor's name and address
State Displays the state that the investor lives in and purchased the investment for
Investment # Displays the investment number
Offering circular agreement Displays the status of the offering circular agreement, along with the user id performing the action and the date and time the action was taken
Privacy agreement Displays the status of the privacy agreement, along with the user id performing the action and the date and time the action was taken
Electronic signature agreement Displays the status of the electronic signature agreement, along with the user id performing the action and the date and time the action was taken
Investment acknowledgement Displays the status of the investment acknowledgement, along with the user id performing the action and the date and time the action was taken

Click the Funding tab.

Funding tab

When you click the Funding tab, you will see information related to the investment type and how the investor wishes to fund the purchase:

The following fields are available on this screen:


Field Description
Category Displays the selected investment category
Type Displays the selected investment type
Amount Displays the elected investment amount
Pay by Displays the method chosen by the investor to pay the funds
EFT date This field is only visible if the Pay by selection is EFT. Displays the date chosen by the investor to process the funds
Bank name Displays the investor's bank name
Bank phone Displays the bank's telephone number, if provided
Bank account Displays the account number
Routing number Displays the bank routing number
Account type Displays the account type (Checking or Savings)
EFT agreement Displays the status of the EFT agreement, along with the accepting user ID and the date and time that the agreement was accepted


If you wish to edit any of this information, click the Edit button on the toolbar. The field will now activate for editing. Make your desired changes and click Save to continue.


Click the Interest tab.

When you click the Interest tab, you see this screen:

This screen shows the selections that the investor made related to interest distribution. If you wish to modify, you can click Edit. When you are done making changes, click Save.


You are now able to approve or deny the form. Click your selection. The form now appears as such on the Review/Approval screen:
