The Health Care Policy Inquiry screen allows you to search for policies based on parameters you select.

The following fields are available:

Field	Description
Policy num	To locate a policy number, enter it in this field and click Search
Start dt	Enter the date in mm/dd/yyyy format or click the calendar icon to select the date
End dt	Enter the date in mm/dd/yyyy format or click the calendar icon to select the date
Remitter PIN	To search by Remitter PIN, type the PIN here or click the search icon to launch the Name/Address Inquiry screen
Insured PIN	To search by Insured PIN, type the PIN here or click the search icon to launch the Name/Address Inquiry screen
Owner PIN	To search by Owner PIN, type the PIN here or click the search icon to launch the Name/Address Inquiry screen
